ALL ATHLETIC PAPERWORK FOR THE 2017-18 SCHOOL YEAR CANNOT BE SIGNED UNTIL JULY 1, 2017
Please read the following to complete your Athletics Online Registration
** Winter Sports Paperwork due by Wednesday, October 25, 2017 - NO EXCEPTIONS. **
Three items to get your student signed up for athletics:
1-Register your child at the Booster Club website (www.gomavericks.org)
2- Submit HARD copies of Health Forms - physical and student accident insurance to your child's coach (NOT to AD or Athletic Trainer)
3- Complete REQUIRED NCHSAA online paperwork by Clicking on the Athletic Team Registration ICON below

Wrestling to Coach Stack
Girl's Basketball to Coach Ellis
Boy's Basketball to Coach Haines
Swimming and Diving to Coach King-Pierce
Indoor Track and Field to Coach Starr
Step 1: Submit Health Forms in Paper Copies (Pre-participation physical examination (2 pgs) and Student Accident Insurance Form)
All Athletic Preparticipation Physical Examination Forms and the Student Accident Insurance Form must be submitted as a hard copy.
*Physicals are good for 395 days.*

Students registering for Athletics must have a current physical examination on file in the athletic director's office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to your coach no later than 7 school days prior to the first day of practice/tryouts.
Physicals completed after 1/1/16 are valid for 395 days from date of the last exam. Download the physical form by clicking the physical form tab at the top.
